Weeping Pink Dahlia: A case of Eosinophilic Gastroenteritis, Serosal Subtype

Significance: Eosinophilic gastroenteritis (EGE) is an inflammatory disorder of the gastrointestinal (GI) tract characterized by eosinophilic infiltration of the bowel wall, because the symptoms are very similar to other GI disorders, EGE has been often underdiagnosed and thus considered a rare disorder

Clinical Presentation: This a case of a 24/F with no known co morbidities and no known allergies, who had a 2 week history of on and off crampy abdominal pain, watery stools occurring 3 to 4 episdoes per day amounting to 3 glasses per episode; no associated fever, overt bleeding or weight loss. Ultrasound done at the outpatient revealed presence of moderate ascites.

Management: CT scan done revealed a long segment bowel wall thickening from the duodenum until ileum and moderate ascites. Halo sign and arnaeib limb like sign were also appreciated. Upper GI endoscopy with gastric and duodenal biopsies showed mucosal erosions in the gastric and duodenal mucosa. Histopathology revealed mucosal eosinophilic infiltration (almost 100/hpf). Patient’s IgE is normal and food allergy panel is negative. Steroids and PPI were started. There was noted resolution of symptoms after steroid initiation.

Recommendation: Eosinophilic GI disorders are considered rare disease conditions but can also be due to under reporting because of its non specific Gastrointestinal presentation. Clinicians must have a high index of suspicion and include EGE as one differential especially in patients with peripheral eosinophilia.
