A Case Series of Duodenal and Pancreatic Metastasis from Renal Cell Carcinoma: Unusual locations of gastrointestinal metastasis

Background: Renal cell carcinoma (RCC) metastasizing to the duodenum and pancreas are rare and unusual sites of metastasis with literature listing only a handful of case reports.

Clinical Presentation: We report two cases of duodenal and pancreatic metastasis of renal cell carcinoma, respectively. The first case is a known case of stage II RCC presenting with melena and weight loss 13 years after radical nephrectomy. Computed tomography revealed hepatic and pancreatic masses. On upper endoscopy noted a friable mass at the first segment of the duodenum where a biopsy taken was consistent with metastatic RCC. The second case is a known RCC case who presented with vague abdominal discomfort ten years after radical nephrectomy. Positron emission tomography revealed a pancreatic head mass. Biopsy taken of the said mass via endoscopic ultrasound revealed metastatic RCC. Both patients underwent palliative radiotherapy.

Conclusion: In patients who have underwent nephrectomy for RCC, metastasis to gastrointestinal sites must be kept in mind especially in those with gastrointestinal symptoms so that a thorough workup is not missed and endoscopic investigation can be carried out.
