The Role of N-acetylcysteine in a Filipino with alcohol-associated Hepatitis in Acute Liver Failure

Significance: Severe alcohol-associated hepatitis has a high mortality rate. It is currently recommended to administer glucocorticoid therapy in these patients. However, glucocorticoids may be contraindicated in some cases such as concomitant infection, acute kidney injury such as in this case. Due to this, it is deemed beneficial to have effective alternatives to steroids. Intravenous N-acetylcysteine, on the other hand, is often used empirically as a treatment for Drug-Induced Liver Injury, most especially acetaminophen.

Clinical Presentation: We report a case of a 43-year-old Filipino male who presented with seizures, jaundice, and stupor. He was diagnosed as a case of alcohol-associated hepatitis in acute liver failure. The patient received intravenous N-acetylcysteine, which resulted in rapid improvement in clinical status and in liver enzymes. He was sent home stable and fully recovered.
