PANC-it WITH EGGS CROSSTALK: A CASE REPORT OF PANCREATIC MALIGNANCY WITH OVARIAN METASTASIS

Clinical Presentation: Pancreatic cancer is the fourth leading cause of cancer-related death and has a dismal prognosis. It tends to metastasize through the lymphoid system to organs such as the lung, liver, bone, and spleen. 1 However, pancreatic cancer with ovarian metastases is rare. Pancreatic cancer that has metastasized to the ovaries is found in 4%-6% of patients during autopsy but it is rarely diagnosed clinically. Patients with an ovarian metastasis generally have no distinguishing clinical symptoms or signs. 2 Here we present an interesting case of a 40 year old Filipino female with a two month history of bloatedness, early satiety, abdominal enlargement and undocumented weight loss; however, there was no noted history of vaginal bleeding or hypogastric tenderness. Abdominal imaging revealed a pancreatic mass, highly considering a pancreatic malignancy. She underwent an exploratory laparotomy to biopsy the pancreatic mass, however, during the procedure, there was also noted masses involving both ovaries which prompted a bilateral salpingo-oophorectomy. Histopathology results showed an adenocarcinoma involving the pancreas and ovaries. The final diagnosis was pancreatic carcinoma with ovarian metastasis. It is necessary to determine the site of primary malignancy for appropriate treatment and a chance for decrease mortality among patients. Due to its poor prognosis and high mortality rate, early detection and diagnosis would be beneficial to patients for a better chance of overall survival.
