Abstract
Hepatocellular Carcinoma in a Noncirrhotic Accessory Liver Lobe

Clinical Presentation: This paper presents a case of a 29-year-old Filipina who came in with epigastric pain and nausea. Ultrasound showed a liver mass, later confirmed by a contrast computed tomography scan (CT scan) showing an enlarged left lobe of the liver with confluence of masses aggregately measuring 14.6 cm x 21.2 cm x 14cm involving segments 2, 3, 4, 5, and 8. Core needle biopsy findings revealed atypical hepatocytes in trabecular growth pattern with hyperchromatic, moderately pleomorphic nuclei surrounded by moderate granular eosinophilic cytoplasm. Patient was advised extended left hepatectomy. However, intraoperative findings revealed otherwise. A 20 cm x 18 cm firm mass adjacent to the lesser curvature of the stomach and adherent to the mesentery with a ligament connecting to the liver was found to be consistent with an ALL. Due to unexpected intraoperative findings, initial plan of extended hepatectomy was revised to a simple resection of ALL. Final biopsy was consistent with hepatocellular carcinoma. The finding of an Accessory Liver Lobe is interesting in itself given the rarity of this condition. Even more interesting is the occurrence of hepatocellular carcinoma in this rare entity.
