Significance: Mucinous adenocarcinoma is a rare disease entity accounting only of 0.4 to 1% of all gastrointestinal malignancies. In general, mucinous neoplasms of the appendix are diagnosed histologically and are classified as low-grade, high-grade appendiceal neoplasm or adenocarcinoma of the appendix depending on stromal invasion. The management of these neoplasms depend of the stage of the disease, and may include surgery and Hyperthermic Intraperitoneal Chemotherapy. This paper presents a case of a 37-year old patient with a large 15x20cm irregular, lobulated, ulcerating mass at the level of the umbilicus extending inferiorly to the mid-thigh. Contrast computed tomography (CT scan) revealed a huge, low density mass in the right lower quadrant protruding though the skin measuring approximately 18cm x 20cm with pseudomyxoma peritonei. Histopathology was consistent with mucinous carcinoma of the appendix. Due to its inoperability, the patient was advised for systemic chemotherapy.
