Abstract

RADIOFREQUENCY VERSUS MICROWAVE ABLATION OF HEPATOCELLULAR CARCINOMA IN HIGH-RISK LOCATIONS

Objectives: Locoregional interventional therapies play a leading part in the management of hepatocellular carcinoma (HCC). This study aims to compare treatment outcomes of radiofrequency ablation (RFA) versus microwave ablation (MWA) in patients with HCC in high-risk locations.

Methodology: This is a retrospective study which included HCC patients who underwent RFA or MWA. We compared the outcomes of high-risk tumors with non high-risk tumors in terms of complete ablation and recurrence rates. We also identified the predictors of survival and recurrence between RFA and MWA groups.

Results: The study included 192 HCC patients with 622 tumors in total, in which 367 were considered as high-risk. Despite differences in tumor characteristics (size p=0.002 and number p=0.001), there was no significant difference in the complete ablation rates for RFA vs MWA in high-risk tumors (92.1% and 91.7%) versus non high-risk tumors (92.5% vs 97.4%). Predictors of survival include presence of solitary tumor (p=0.008), Child Pugh score of A (p=0.025), and absence of recurrence (p=0.44). Whereas predictor of recurrence include multiple tumors (p= 0.01).

Conclusion: Radiofrequency and microwave ablation have similar efficacy profiles for the treatment of hepatocellular carcinoma in both high-risk and non-high-risk locations
