Abstract
Association of Triglyceride-Glucose Index and Glycosylated Hemoglobin with the Degree of Fibrosis in Patients with Non-Alcoholic Fatty Liver Disease Evaluated by Acoustic Radiation Force Impulse Imaging

Background: Non-alcoholic fatty liver disease (NAFLD) has emerged as the most common cause of chronic liver disease worldwide. It encompasses the clinical spectrum of liver diseases that could lead to progressive hepatic fibrosis and cirrhosis. Because of this, there is a need to identify subjects with NAFLD who have early signs of hepatic fibrosis.
Methodology: A retrospective analytical study was used to determine the correlation of TyG (Triglyceride-glucose index) and HbA1c (Glycosylated hemoglobin) to the degree of hepatic fibrosis in NAFLD evaluated using acoustic radiation force impulse (ARFI) imaging. Correlation was confirmed using t-test for two independent variables and analysis of variance. To accurately identify predictive values, a combination of univariate binary logistic regression and receiver operating characteristic was applied.
Results: A total of 186 subjects with mean age of 56.5 years were included in the study. Using ARFI imaging, 96 of 186 (51.6%) subjects had METAVIR score of F2. The clinical and biochemical characteristics of patients in different stages of fibrosis showed that HbA1c level, TyG index, and fasting blood sugar level were higher in advanced fibrosis. Furthermore, using univariate binary logistic regression, the odds ratio for TyG index and HbA1c level increased accordingly to the unit increase. Lastly, computed cut-off value of TyG index was 4.73 while that for HbA1c level was 6.54% suggesting advanced fibrosis.
Conclusion: TyG index and Hba1c have a significant association with the degree of hepatic fibrosis in patients with NAFLD. These variables are effective in identifying individuals at risk for liver fibrosis.
