CLINICAL PROFILE AND OUTCOME OF PATIENTS WITH HEPATIC TUBERCULOSIS

Significance: Available local data on hepatic tuberculosis is limited and potentially outdated. Recent guidelines included case definition and ancillary tests that were not considered in previous studies. We aimed to provide an updated clinical profile and outcome of patients with hepatic tuberculosis.

Methodology: This is a retrospective chart review study where clinical and laboratory parameters were determined from patients with hepatic tuberculosis seen in a tertiary hospital in Manila from January 2007 to December 2022. Statistical analysis was carried out using SPSS Version 25.

Results: A total of 40 patients were included of which 23 (58%) and 17 (55%) were bacteriologically and clinically diagnosed, respectively. Concomitant liver disease (10%) is low, while biliary tree involvement is not uncommon (32.5%). Abdominal pain is the most common clinical presentation (42.5%), followed by weight loss (37.5%) and jaundice (32.5%). Hepatic mass or nodule is the most common imaging finding in ultrasound (77.5%) and CT scan (82.1%). A third of patients were lost to follow up (32.5%). Another third had clinical improvement (30%), while the rest either deteriorated (22.5%) or were still ongoing treatment (15%). The average duration of treatment was 8 months.

Conclusion: Clinical diagnosis of hepatic tuberculosis, including classic symptomatology and imaging findings, need to have a more robust and universal definition. The recommended duration of treatment of 6 months for hepatic tuberculosis may also need to be revisited. Stronger strategies to increase compliance with medication and follow up must be developed as well.
